Abstract

Intramedullary interlocking nailing is widely used for extra-articular distal tibial fractures, yet outcome interpretation depends on consistent definitions and transparent reporting. Purpose: To provide a critical appraisal of Dake et al.’s cohort study on IMN for EADTFs. Main Points: We call for an explicit, standardized definition of “distal” (distance from the tibial plafond and/or AO/OTA classification), clearer reporting of outcome assessment timing, and incorporation of validated patient-reported measures beyond AOFAS/OMAS to capture late sequelae, including anterior knee pain. We also encourage detailed perioperative protocols (weight-bearing progression, rehabilitation pathway, surgical approach, and open-fracture wound care) to improve reproducibility and clinical translation.
